Required Health Forms

Health Forms must be completed annually for each student enrolled at Greensboro Day School. 

All new students must provide an immunization record to your child's division prior to the beginning of the school year. Immunization records can be obtained from your pediatrician's office or your child's previous school.

Health Forms include:
  • Health Information (NEW online form that will be emailed in mid-July) *Required for all students grades BB-12.
  • Early Childhood and Kindergarten health assessment form *Required for students grades BB-K.
  • Medication Administration form *Required for all students grades BB-12 and requires your pediatrician's signature.
  • An Immunization record is required for all new students, all incoming Kindergarten students, and students who had a change to their immunization record.
  • Please complete allergy and anaphylaxis care and diabetes care if needed

Bengal Bookstore Hours

The Bengal Bookstore is open Monday through Friday, 8 a.m. - 1 p.m. and  2 - 4 p.m.

Attractions Guide

Save the date for the Attractions Guide Fundraiser this fall! Sale dates are August 29 - September 19. Proceeds support the many projects of the Parents' Association including updates to the lower school technology lab, middle school furniture, and counseling area in the upper school. 2018-19 Tuition
